2011-2012 Registration Forms

Below are the forms you will need to register your child. They are updated from previous years, so please take a moment to identify the changes. One important change is the Registration Contract located at the bottom of the page. Please read through the outlined policies and sign the bottom in order to register your child for classes.

Also, you will notice that each child needs their own form. Please use the standard form for your first child, and the ADDITIONAL CHILD registration form for any other children you are registering for classes.
